Health Practitioners Have Ethical Responsibilities

Your health practitioner has an ethical responsibility to keep your care as the top priority. For example, if you go to see your therapist for a pain in your hip and they have you come back every week for three months but your problem really is not getting any better.

Do you consider this ethical? I do not.

I have ethical parameters that I have set for my practice. That is, if my clients are not seeing a reduction in symptoms and an improvement in their condition within three treatments, I will either think about what I am not doing and try something else. Or I will send them to another practitioner in my network for a second opinion and some alternative treatment.

This is my ethical responsibility to you to keep your care as the top priority, and it should be a responsibility for all health practitioners as far as I'm concerned.

The reason I want you to keep these things in mind is that, I have seen many people going to a health practitioner trying to overcome a problem, only to be going there for six months or more and not getting any results. This is not right. As I've pointed out above, your therapist or practitioner has an ethical responsibility to you.

Ask Questions When You Are Not Seeing The Results You Should

If you are not getting the results that you want within a reasonable time frame, then you should be asking your health practitioner's motives. Are they just trying to wean more money out of you? Are they scared to lose you as a customer if they refer you on to someone else? Do they simply not know what to do or how to treat your condition properly?

The thing is, no practitioner knows everything. We all have our limitations and that's okay. As long as we know when and where it is time to send our clients elsewhere, or seek another opinion. Do not get locked into having treatment that you feel is not working. And do not feel guilty or bad about saying something to your practitioner if you want an explanation, a better result, or more feedback.

You pay good money to have a good service delivered to you, so make sure your health practitioner is operating with ethical principles and can provide you with a reasonable estimate in the time frame it is going to take for you to get better. What the chance of recovery are for you. If there are other things you can do to help progress your recovery quicker. Just be sure that your remedial therapist or health practitioner has your care as a top priority.

This is a very important first point because if you are looking for someone who specializes in fixing chronic pain, then you will not select a therapist that works in a spa. Why? Because usually these types of therapists only do relaxation massage and therefore they will not be able to help fix your pain.

Be clear about what you want so you know the kind of income you want to receive from your treatment. Then you can look around for a therapist and make a short list of possible therapists that can give you the treatment you want.

Could Your Computer Be Hurting You?

In today's world we are hooked on computers, myself included. Most people spend their whole work day on the computer and then come home and play on it more for personal fun and social networking sites like one of my favorites, Facebook. The downfall of all this time on the computer is that we do not take the time to be aware of our posture using the computer, nor do we take a moment to step away and give our body and minds a much needed break, even if it is just for 5 minutes. With this, most of us find ourselves in a bit of pain from the all day use of the keyboard and the mouse. This pain could be in your upper shoulders, your forearms, wrists and for some in the fingers (ever feel that numb sensation). Sometimes these muscle issues start showing up right away. Sometimes they may not be for years.

I'm sure if you take a good look at your computer set-up and your posture, you'll quickly realize that things are not ergonomically correct. There are some ways to help yourself have better posture and be more ergonomically correct. These simple measures can help deter you from compromising your body.

  1. Receive regular massages
  2. Acquire ergonomically correct products to support your wrists, arms, lower back and even for your legs.
  3. Take breaks away from the computer. Even a quick 1 to 2 minute walk down the hall will refresh your body, get the blood flowing and even refresh your mind.

    • Deltoid Stretch: Pull the right arm across the front of the body using the left arm. Make sure you stretched arm has the thumb facing down. Hold and then switch arms.
    • Tricep and Lat Stretch: Point your right elbow toward the ceiling, then lean the upper torso to the left. You may grab the elbow with the left hand for a further stretch. Hold and switch arms.
    • Wrist Stretch: Hold arm straight out with hand upside down. Gently pull down on the fingers to stretch. Switch hands.

I'm sure one of your first thoughts is how could I fit this into my day? Or I can not do these, what if the boss sees me and thinks I'm being unproductive? Please realize that these simple things do not take up much time and will help you be more productive. If your body is not in pain you will not be spending the time complaining about it, you will get more done and you'll feel better. In the end it is your body and you only have one chance to take care of it.

Massage Just for the Wealthy – Invest in Yourself

Many people look at massage as a luxury, something to treat yourself with once a year for your birthday or anniversary, or even something that only the 'wealthy' do. I've had many people ask, or even state and assume that my clients must only be wealthy clients. Their expectation is that only the 'rich' can afford massage. If that's the case, then why are they so many spas and massage therapists? There can not be that many 'rich' people keeping these places in business can there? Ya, I do not think so.

Massage is for most everyone including kids and animals (and yes I've massaged kids, dogs, cats and a horse). It does not matter what your checking account balance is, it's a whole variety of people that get massages. For my business I have the range of clients that yes others would consider wealthy. I also have middle-income as well as the other end of the spectrum of clients that live solely on disability and social security income. Would they be considered wealthy? Not in a financial sense, no. In a healthy, well-being sense? Absolutely! And that goes for all my clients, the amount of money they have does not factor into the massage I do. I consider every client I have wealthy in who they are and the type of person they are. There is a quote I heard a while ago that I think relates to this topic: First Wealth is Health ~ Ralph Waldo Emerson.

Massage is something people do for themselves, it is an investment into their own health, something that you do for you to help keep your body and mind in check. What one person may consider expensive another does not. I may consider paying $ 60 or more for pants expensive while someone else freely spends their money on $ 100 pants. Part of it really comes down to what you want to spend your money on. Do you want to spend money on you? Do you think you're worth the cost of massage? Do you think your health is worth it? Well you should think you're worth it, because you are! What I really think it brings down to is that we are taught not to ever be selfish and we consider massage a selfish act. Again, it's not and your first wealth is your health and it's an investment into you! So go ahead, make that investment! Are not you worth it?

A Common Cause of Low Back and Sciatic Pain – The Piriformis

As a licensed massage therapist, I specialize in chronic pain and injury management using deep tissue massage therapy. Most of my clients come in with a common complaint: pain in the low back, hips (primarily on one side), and / or shooting pain or numbness down the back of the leg.

More than two million Americans suffer either from back pain or a form of sciatica that is often misdiagnosed and improperly treated. Many suffer from pain for months or years without a proper diagnosis. I work with these symptoms so frequently that I have noticed some common characteristics with this all-too-common pain complaint.

The most common character I find are aggravated trigger points (TrPs) in the piriformis muscle. Piriformis trigger points are often confused for a herniated disc, sciatica, or other back issues, and many sufferers undergo unnecessary and costly tests, injections, and surgeries.

What is the piriformis?

The piriformis muscle is a small external rotator of the hip whose function primarily is to turn the knee and foot outward. It lies deep within the gluteal muscles, originates from the sacral spine, and attaches to the greater trochanter of the femur – the big bony “bump” on the outside top of the thigh. The sciatic nerve passes benefit through an opening called the sciatic notch.

Function

In non-weight bearing activities, foot unloaded and hip extended, the piriformis rotates the thigh outward. When the hip is flexed at 90 degrees it aids in hip abduction.

In weight bearing activities, foot loaded, the pirifomis is often needed to control rapid medial rotation of the thigh – for example, as the foot strikes the ground during walking or running, the knee turns inward.

Piriformis Trigger Points (TrPs)

Symptoms

The myofascial pain component includes pain in the low back, groin, buttock, and hip. A TrP may cause the muscle to compress and irritate the sciatic nerve, causing the pain to travel along the course of the nerve. The pain may radiate down the back of the leg and into the hamstrings, the calf muscles, and possibly the foot. The pain may initially be confused with a hamstring strain. Weakness, stiffness and a general restriction of movement are also quite common. Tingling, numbness, or shooting pains down the leg can also be experienced. Symptoms tend to be aggravated by prolonged sitting or by intense activity.

Causes

Piriformis TrPs are predominately caused by a shortening or tightening of the piriformis muscle. Pirimformis TrPs are commonly associated with sports that require a lot of running, change of direction, or weight-bearing activity. Pirimformis TrPs can result from acute overload (as when catching oneself from a fall), from repetitive overload (as with the rapid internal rotation of the weight bearing experienced by walkers and runners with poor biomechanics), or from sustained overload (as when holding the leg bent and turned out for prolonged periods while driving a car or working at a desk). Sciatic pain from piriformis TrPs is also a common complaint during pregnancy.

The piriformis muscle is responsible for the symptoms of pain by projecting pain from activation of the TrPs and by nerve entrapment upon the sciatic nerve. Once TrPs are activated, the piriformis muscle begins to put pressure on the sciatic nerve. The sciatic nerve runs under (and sometimes through) the piriformis muscle on its way out of the pelvis. The piriformis muscle can squeeze and irritate the sciatic nerve in this area, leading to the symptoms of sciatica.

Treatment and Prevention

The first two steps in treating piriformis TrPs can provide the most significant and immediate relief: deep tissue massage with stretching of the external rotators of the hip. Deep tissue massage techniques such as myofascial release and trigger point therapy are highly effective at reducing active trigger points. Once the TrP is released, there will be a significant reduction in pain.

Following up the massage with flexibility training will help loosen the muscle and help prevent a return of the TrP. I have found that it takes between four and ten massage sessions to move out of the acute pain phase from piriformis TrPs. It may be shorter or longer depending on the cause and severity of the initial injury.

For long-term prevention of piriformis TrPs, self massage and flexibility will be your most invaluable tools. Use a ball (preferably a tennis ball for beginners) for self trigger point therapy, and stretch the piriformis muscle and other external rotators of the hip. With each stretch it is important to breathe into the stretch and only stretch to slight discomfort … NOT PAIN.

What is causing your pain?

This is a question I attempt to answer many times each week. My most common answer is:

DURATION X REPETITION = PAIN When movement hurts, it can usually be traced back either to a repetitive movement pattern or a repetitive lack of movement pattern – how much time you either repeat a single movement or maintain a specific position. Both repetitive movement and lack of movement create pressures within the fascial system.

Fascia is a three-dimensional continuous web of connective tissue that wraps around your body from the bottom of your feet to the top of your head. Fascia surrounds your muscles, tendons, ligaments, bones and organs. Fascia is the organ system that gives your body shape and plays a fundamental role in your posture.

The way you move each day determines the shape and well-being of your fascia and the structure it supports, your body. If, for instance, you sit for long hours each week (duration and repetition), the fascia in your hips, shoulders, and neck will begin to form adhesions at the folds of your fascia in these positions. These adhesions impinge upon your body's “working parts;” bind the head, shoulders, and pelvis into a forward position; create trigger points; and restrict range of motion.

Over time and repetition, these adhesions become semi-permanent obstacles that change your body's shape. The change in shape shifts the balance of the body, placing stress and strain through. This leads to the intensity, discomfort, and pain that many people feel in their back, knees, neck, or shoulders.

Given sufficient time and repetition, it may lead to more serious structural injuries, ie, tears of the soft tissue in the shoulder, herniation of vertebrae, or wearing down of the cartilage in the knees and hips.
